The authors identified 34 studies involving more than 21,000 women that compared policies to induce labor, usually after 41 completed weeks of pregnancy, with waiting for labor to begin, or waiting for a period before labor was induced, or both. The studies took place in Austria, Canada, China, India, Finland, Malaysia, the Netherlands, Norway, Russia, Spain, Sweden, Thailand, Tunisia, Turkey, Great Britain and the USA.
Babies in hospitals that had a policy of inducing labor after a certain time compared to waiting for labor to start naturally had fewer deaths. The induction groups had fewer intensive care nursery admissions and fewer low Apgar scores five minutes after birth (a simple health test for babies) compared to the waiting period.
There was less need for caesarean section with induction compared with waiting, with similar rates for vaginal delivery regardless of whether labor was present or not.

The authors found no clear differences between waiting for labor to start naturally versus policies encouraging birth at 37 weeks or later: risk of perineal trauma to mothers; bleeding after childbirth; the length of the mother’s stay in hospital; breastfeeding; in infants with early abnormal neurological function (encephalopathy); or birth trauma.
Most of the included studies were well conducted and well reported. The authors’ confidence (confidence) in the evidence is generally medium to high.
A policy of induction of labor compared with waiting for labor to begin is associated with fewer infant deaths and fewer cesarean sections; with no clear difference in vaginal delivery.
